Passport Information
The City of Beaver Dam is now an official Passport Acceptance Facility. Applicants may apply for passports at City Hall Monday thru Friday from 7:30 a.m. to 3:00p.m. For questions concerning passports, please call 270-274-7106.
To apply for a passport you must bring the following items with you:

Original birth certificate or a certified copy

Photo ID such as driver's license, military or school I.D.

2 passport photos (our local Wal-Mart offers this service)

2 forms of Payment (Must be submitted in the form of a check or money order. Please call the office for payment amounts.)
**Children under the age of 16 must be accompanied by both parents. In the event of sole custody the appropriate paperwork must be provided.
